You will find Malus Thorm in Act 2 in House of Healing, which is located at the following coordinates: X:-200, Y:35. Once inside, you can see the boss surrounded by nurses, who are about to perform another operation on the unfortunate victim. After a short dialogue, you will have two options for what to do next: fight or engage in a dialog with the tyrant. If you prefer the second option, we will start with it.

If you decide to go with the easier and more effective option, you will need to choose a character with the highest Charisma skill. Here are a few options for suitable companions:

The dialog options we’re about to describe can either kill the boss or make it easier for you to fight him.

Convince Malus Thorm to Kill the Nurses and Himself

You’ll need to make Religion and Persuasion checks for this dialog option, so make sure you choose the right character. Here are the dialog options to choose from:

“Recall what you know of Shar’s teachings” – here you will have to make a DC 14 Religion Check.
“The sisters aren’t ready. They’ll make me sick instead of curing me.” – In this case, you will need to make a Persuasion check with a DC 16. After a successful roll and convincing Malus Thorm, he will agree with you and ask how to implement it.
“Why not have them hone their skills on each other?” – This time you’ll need to make another Persuasion check, with a DC 21. The nurses will start fighting each other and dying, leaving Malus Thorm alone with you. If you convince Malus Thorm to do so, you can ensure an easier fight with the boss even if you fail the last dice roll, as you have eliminated his assistants.
Malus Thorm will get angry because his nurses are dead and will want to kill you. But fortunately, you will have another dialog option available to you. BG3: How to Defeat Malus Thorm in Combat

Defeating Malus Thorm in Baldur’s Gate 3 is not difficult, but you shouldn’t forget about the nurses, who can both buff the boss and distract your teammates from directing their attacks at him. In general, there are two main steps to kill Malus Thorm:

Kill Nurses First

We recommend selecting one character to act as a tank to attract the enemy’s attention. The rest of the characters should deal as much ranged damage as possible. The best companions to choose are Shadowheart or Cleric into the fight, then you can use Sacred Flame or Guiding Bolt to deal huge damage to an enemy.

If you didn’t manage to convince Malus Thorm of the nurses’ death option during the dialog, they will be your first target, as they can provide him with various medical tools and weapons, such as the Needle, that he can use to heal himself. Stock up on Drow Poison before the fight, so you can use it to put Malus Thorm to sleep in the first round and gain time to kill his nurses. You can also use Shadowheart or Cleric to cast Turn Undead spells on the nurses, which will prevent them from passing tools and weapons to Malus Thorm.

Kill Malus Thorm

The second step is to fight Malus Thorm itself. Note that this boss is resistant to certain types of attacks, namely: Slashing, Piercing, Bludgeoning, and Fire damage.
