In Baldurs Gate 3, early stages of the game, you have your first encounter with an NPC named Volo at the base of a tree in Druid Grove. He is a highly exaggerated epic writer, and he wants to write about you. After your conversation, he drinks the Invisibility Potionย and leaves to infiltrate the Goblin Camp in order to find something to write about. How to Get Ersatz Eye in BG3

In Baldurs Gate 3, Volo’s Eye Surgery operation to remove the Mind Flayer Parasite from your head fails. Then Volo gives you an Ersatz Eye as an apology in BG3. This Eye in Baldurs Gate 3 allows you to see invisible enemies. This is a permanent effect, and invisible enemies must roll a successful Dice to keep their invisibility intact.
After your conversation with Volo first time in the Druid Grove, he drinks an invisibility potion and goes to Goblin Camp to get information for his writings. At the same time, characters like Rath, and Nettie in Druid Grove want you to go to Goblin Camp to find Halsin. Also, when you clear the Goblin Camp, you will also help the Tieflings to leave the camp.
When you go to the Goblin Camp in Baldurs Gate 3, you’ll see that Volo has been captured, and they’ve made a fool of him. After engaging in the necessary dialogues, one of the Goblins takes Volo away to lock him in his cell. Once you rescue Volo deep inside the castle, you must tell him to wait in the camp.

At the end of Volo’s failed Eye Surgery adventure in BG3, he gives you an Ersatz Eye as an apology. This eye allows you to see Invisible creatures. Long story short, you should definitely let Volo perform Eye Surgery on you and tolerate the disturbing scenes for a few minutes. Later in the game, you may encounter Invisible enemies, and this eye will come in handy.
